1 Dog: Boarding Agreement
2-3 Dogs: Multi Dog Boarding Agreement
Senior Dogs: Senior Dog Medical Release Form in addition to the above.
How to fill out forms & submit
On Desktop: Right click the link and download to your desktop, them open it from there. You should be able to click on each field to fill out the form. When finished save the file, and either click the Submit Button on the last page or you can just attach it to an email & send to dogeday@gmail.com.
On Mobile: Tap to open the PDF. Share Button (square with up arrow), Save to Files. Then open it from the Files App and fill out the form. When finished tap the Share Button (square with up arrow) on your mobile device then tap Email and send it to the same email above.
